Social Worker (LSW Required)

Full-Time | Bethany Home – Sioux Falls, SD
$57,000 – $65,000/year (based on experience)

At Bethany Home, we are caring people caring for people—and we’re looking for a compassionate, motivated Licensed Social Worker (LSW) to join our team.

In this role, you’ll partner with residents, families, and the care team to support each resident’s emotional, social, and psychosocial well-being while ensuring quality care and smooth transitions.

What You’ll Do:

• Complete social service assessments and develop individualized care plans
• Participate in care conferences and collaborate with the interdisciplinary team
• Coordinate admissions, discharges, and transfers
• Provide emotional support and guidance to residents and families
• Connect residents with community resources and services
• Maintain accurate, compliant documentation
• Advocate for resident rights, dignity, and quality of life

What We’re Looking For:

• Bachelor’s or Master’s degree in Social Work
• Active LSW in South Dakota (or ability to obtain quickly)
• Experience in long-term care, healthcare, or discharge planning preferred
• Strong communication and organizational skills

• A team-first mindset and passion for senior care
